Embora comumente associada ao processo de envelhecimento, ela tamb\u00e9m pode ocorrer em pessoas com doen\u00e7as cr\u00f4nicas, hospitaliza\u00e7\u00f5es prolongadas, inatividade f\u00edsica ou desnutri\u00e7\u00e3o. Reconhecida oficialmente como doen\u00e7a pela Organiza\u00e7\u00e3o Mundial da Sa\u00fade desde 2016 (CID-10-CM, c\u00f3digo M62.84), a sarcopenia est\u00e1 associada a riscos elevados de quedas, fraturas, perda da independ\u00eancia, interna\u00e7\u00f5es recorrentes e mortalidade precoce.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A origem da sarcopenia \u00e9 multifatorial. Envolve altera\u00e7\u00f5es hormonais, como a queda da testosterona e do horm\u00f4nio do crescimento, inflama\u00e7\u00e3o cr\u00f4nica de baixo grau, disfun\u00e7\u00e3o mitocondrial, deteriora\u00e7\u00e3o das conex\u00f5es nervosas com os m\u00fasculos e um fen\u00f4meno chamado resist\u00eancia anab\u00f3lica &#8211; quando o corpo responde menos \u00e0 ingest\u00e3o de prote\u00ednas ou ao est\u00edmulo do exerc\u00edcio f\u00edsico.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Para evitar esses desfechos, \u00e9 fundamental reconhecer a sarcopenia o mais cedo poss\u00edvel. E hoje, felizmente, existem formas pr\u00e1ticas e acess\u00edveis para fazer isso. As diretrizes mais atuais, elaboradas por grupos de especialistas internacionais como o European Working Group on Sarcopenia in Older People (EWGSOP2) e o AsianWorking Group for Sarcopenia (AWGS), recomendam uma abordagem em etapas, combinando question\u00e1rios, medidas f\u00edsicas simples e exames complementares, quando necess\u00e1rio.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">O primeiro passo pode ser um rastreamento r\u00e1pido com o question\u00e1rio SARC-F, composto por cinco perguntas sobre for\u00e7a, dificuldade para levantar da cadeira, subir escadas, quedas e necessidade de ajuda nas atividades di\u00e1rias. Uma pontua\u00e7\u00e3o igual ou superior a 4 sugere risco de sarcopenia. Para melhorar a sensibilidade do teste, algumas vers\u00f5es atualizadas adicionam a medida da panturrilha (como o SARC-CalF), o que ajuda a identificar casos mais leves, especialmente em idosos ativos.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A medi\u00e7\u00e3o da circunfer\u00eancia da panturrilha, feita com fita m\u00e9trica comum, \u00e9 uma ferramenta simples, por\u00e9m eficaz. Medidas abaixo de 34 cm para homens e 33 cm para mulheres indicam perda de massa muscular e maior risco de sarcopenia. Trata-se de um m\u00e9todo \u00fatil especialmente em ambientes com poucos recursos ou em atendimentos domiciliares.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Outro marcador importante \u00e9 a for\u00e7a de preens\u00e3o manual, medida com um dinam\u00f4metro. Resultados abaixo de 27 kg para homens e 16 kg para mulheres s\u00e3o considerados baixos e indicam risco aumentado de perda funcional. O Ishii Score, que combina idade, for\u00e7a da m\u00e3o e medida da panturrilha, tamb\u00e9m \u00e9 uma ferramenta pr\u00e1tica e utilizada.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Quando os testes iniciais sugerem sarcopenia, o diagn\u00f3stico deve ser confirmado com a avalia\u00e7\u00e3o da massa muscular por meio de exames espec\u00edficos, como a bioimped\u00e2ncia. Os valores de corte geralmente aceitos para a massa muscular apendicular ajustada pela altura s\u00e3o inferiores a 7,0 kg\/m\u00b2 para homens e 5,5 kg\/m\u00b2 para mulheres.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Al\u00e9m disso, a avalia\u00e7\u00e3o do desempenho f\u00edsico ajuda a classificar a gravidade da condi\u00e7\u00e3o. A velocidade da marcha (caminhada), quando inferior a 0,8 metros por segundo ou escores baixos em testes como o Short Physical Performance Battery (SPPB) indicam sarcopenia avan\u00e7ada e maior risco de perda funcional.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Embora a sarcopenia tenha causas complexas, a boa not\u00edcia \u00e9 que ela pode ser tratada e, em muitos casos, prevenida. A principal estrat\u00e9gia terap\u00eautica \u00e9 o exerc\u00edcio resistido, ou seja, atividades que envolvem for\u00e7a e resist\u00eancia muscular, como muscula\u00e7\u00e3o leve a moderada, exerc\u00edcios com el\u00e1sticos, pilates ou mesmo atividades funcionais adaptadas. O ideal \u00e9 que o plano de atividade f\u00edsica seja individualizado, progressivo e, se poss\u00edvel, acompanhado por um profissional capacitado.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A nutri\u00e7\u00e3o adequada \u00e9 outro pilar essencial no enfrentamento da sarcopenia. A ingest\u00e3o de prote\u00edna de alta qualidade deve ser mantida (em geral, recomenda-se entre 1,0 a 1,2 g de prote\u00edna por quilo de peso corporal por dia, podendo chegar a 1,5 g\/kg em idosos fr\u00e1geis). A vitamina D, quando insuficiente, deve ser suplementada, pois tamb\u00e9m influencia diretamente na for\u00e7a muscular e no equil\u00edbrio. Outras subst\u00e2ncias, como leucina, HMB ou creatina, v\u00eam sendo estudadas, mas ainda n\u00e3o s\u00e3o consideradas padr\u00e3o de cuidado. Da mesma forma, tratamentos farmacol\u00f3gicos espec\u00edficos ainda est\u00e3o em fase de pesquisa e n\u00e3o s\u00e3o recomendados rotineiramente.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">A sarcopenia ainda \u00e9 pouco conhecida fora dos c\u00edrculos cl\u00ednicos, mas deveria fazer parte das conversas sobre envelhecimento saud\u00e1vel. Afinal, o m\u00fasculo \u00e9 muito mais do que est\u00e9tica ou for\u00e7a: ele est\u00e1 diretamente ligado \u00e0 nossa capacidade de andar, levantar da cadeira, manter o equil\u00edbrio, evitar quedas e permanecer independentes. Detectar e tratar precocemente a sarcopenia pode fazer a diferen\u00e7a entre envelhecer com autonomia ou com limita\u00e7\u00f5es evit\u00e1veis.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Profissionais de sa\u00fade, cuidadores, familiares e os pr\u00f3prios idosos devem estar atentos aos sinais sutis dessa condi\u00e7\u00e3o. As ferramentas de rastreamento est\u00e3o cada vez mais simples, eficazes e dispon\u00edveis e a interven\u00e7\u00e3o precoce \u00e9 altamente eficaz.
